SENIOR HIGH SCHOOL GRADUATES

  1. Original and Duplicate copy of Form 138 (Grade 12 Report Card)
  2. Original and Duplicate copy of Certification of Good Moral Character
  3. Original Columban College Classification Test (CCCT) Result
  4. Duplicate copy of Baptismal and PSA Birth Certificate
  5. Three (3) copies of recent picture (2x2)
  6. Duplicate copy of Marriage Certificate, if married

TRANSFEREES

  1. Original Copy of Certificate of Eligibility to Transfer (Transfer Credential)
  2. Original and Duplicate copy of Transcript of Records for Evaluation or a Copy of Scholastic records showing the subjects, number of units taken, and the grades obtained.
  3. Original and Duplicate copy of Certification of Good Moral Character
  4. Original Columban College Classification Test (CCCT) Result
  5. Duplicate copy of Baptismal and PSA Birth Certificate
  6. Three (3) copies of recent picture (2x2)
  7. Duplicate copy of Marriage Certificate, if married

CROSS-ENROLLEES

  1. Cross-Enrollment Permit

FOREIGN STUDENTS

  1. Original Transcript of Records / Scholastic Records / Certificate of year level completed from the last school attended.
  2. Letter of Recommendation from the school last attended.
  3. Certificate of Eligibility to Transfer if previously enrolled in Tertiary Philippine School.
  4. Original and Duplicate Copy of ACR I-Card / Valid Visa for Study
  5. Original and Duplicate copy of Passport
  6. BID documentary Requirements for the conversion of Tourist Visa (9a) into a Student Visa (9f) (Please refer to BI FORM V-NI-005-Rev1)
  7. Original and Duplicate copy of Birth Certificate
  8. Original Columban College Classification Test (CCCT) Result
  9. Three (3) copies of recent picture (2x2)

    Notes:
  • All foreign documents must be original and authenticated by the Philippine Foreign Service Post (PFSP), which has jurisdiction over the place of issuance, or by the Department of Foreign Affairs (DFA) if said document is issued by the local Embassy in the Philippines, with English translation if written in other foreign language.
  • Applicant may be required to submit other documents not included above when needed upon evaluation of admission application.
